ABSTRACT

Objective:To observe the clinical effect of modified Shishi Niubangzi Decoction combined with strengthening muscle-waist exercise on lumbar disc herniation (LDH).Methods:Randomized controlled trial. A total of 60 patients with LDH admitted to the Pinggu Hospital, Beijing Traditional Chinese Medicine Hospital, were enrolled as the research objects between September 2020 and September 2021. According to the random number table, they were randomly divided into the treatment group and control group, 30 in each group. Both groups were given routine basic treatments (strengthening tendons-waist exercise and three-position six-step manipulation). On this basis, the treatment group was treated with modified Shishi Niubangzi Decoction, while the control group was treated with non-steroidal anti-inflammatory drugs (ibuprofen codeine sustained-release tablets). Both groups were treated for 4 weeks. The responsive rates, back pain intensity, leg pain and numbness by Visual Analogue Scale (VAS) and lumbar function by Oswestry Disability Index (ODI), and Japanese Orthopaedic Association (JOA) were compared between the two groups.Results:The response rate of treatment group was significantly higher than that of control group (93.3% vs. 73.3%; χ2=4.32, P=0.038). After treatment, scores of JOA (subjective symptoms, signs, activities of daily living) in the treatment group were significantly higher than those in the control group ( t=3.86, 2.71, 2.21, P<0.05). After treatment, scores of back pain (2.12±0.21 vs. 3.02±0.32, t=12.88), leg pain (2.04±0.64 vs. 2.64±0.66, t=3.58), lower limb numbness (1.75±0.24 vs. 2.41±0.70, t=4.89) in the treatment group were significantly lower than those in the control group ( P<0.01). At 1 week and 1 month after treatment, ODI scores in treatment group were significantly lower than those in control group ( t=10.22, 5.59; P<0.05). Conclusion:The modified Shishi Niubangzi Decoction combined with strengthening tendons-waist exercise can improve responsive rates, improve lumbar pain and function in LDH patients.

ABSTRACT

@#Objective To explore the clinical effect of sling exercise on postpartum low back pain (PLBP). Methods From February, 2017 to February, 2018, 66 PLBP patients were randomly divided into control group (n = 33) and observation group (n = 33). The control group received routine rehabilitation physiotherapy and American chiropractic, and the observation group was supplemented with sling exercise therapy additionally. They were evaluated with Numerical Rating Scale (NRS), Oswestry Disability Index (ODI) and the thickness of transversus abdominis before and six weeks after treatment. Results Before treatment, there was no significant difference in the scores of NRS, ODI and the thickness of transversus abdominis between two groups (P > 0.05). Six weeks after treatment, the scores of NRS and ODI significantly decreased (t > 14.579, P < 0.001), and the transversus abdominis thickness significantly improved (t > 15.855, P < 0.001) in both groups. All the above indexes were significantly better in the observation group than in the control group (t > 4.818, P < 0.001). Conclusions ling exercise therapy, combined with American chiropractic treatment and routine rehabilitation physiotherapy, can further relieve pain in patients with PLBP, and improve the function and the quality of life.

@#Objective To investivate the effects of VocaStim?-Master electrical stimulation and pharyngeal muscles training on sleep breathing parameters, clinical symptoms and cognitive functions in stroke patients with obstructive sleep apnea syndrome (OSAS). Methods From December 1st, 2014 to November 30th, 2017, 50 stroke patients complicated with OSAS were divided into control group (n = 25) and research group (n = 25). All the patients received routine medicine and rehabilitation, while the research group accepted VocaStim?-Master electrical stimulation and pharyngeal muscles training in addition, for four weeks. Their symptoms were observed, and tested with polysomnography (PSG) and Mini-Mental State Examination (MMSE) before and after treatment.Results After treatment, the clinical symptoms improved (P < 0.05), while apnea hypopnea index (AHI) (t > 2.270, P < 0.05)and oxygen desaturation index (t > 3.044, P < 0.01) decreased, and average oxygen saturation (SaO2) and lowest SaO2 increased (t > 3.095, P < 0.01), in the research group, compared with those before treatment and those in the control group (P < 0.05), the scores of MMSE increased (t > 2.859, P < 0.01). There was no significant difference of AHI, averge SaO2, lowest SaO2 and oxygen desaturation index during the period of treatment in the control group (P > 0.05), nor for the score of MMSE.Conclusion VocaStim?-Master electrical stimulation combined with pharyngeal muscles training could effectively ameliorate nocturnal sleep apnea symptom and improve cognitive functions in stroke patients complicated with OSAS.

ABSTRACT

Purpose To investigate the reproducibility and risk factors of aortic distensibility quantification in patients with abdominal aortic aneurysm using multi-slice spiral CT.Materials and Methods The abdominal aortic computed tomography angiography data of 54 patients with infrarenal abdominal aortic aneurysm were prospectively studied.64-muti detector spiral CT,retrospective ECG-gating and segment data collecting scanning were all carried out.The aortic distensibility,D value,and pulse wave velocity at renal artery level and infrarenal artery level were calculated using semiautomatic segmentation software.The difference of aortic distensibility at different levels was compared,and consistency test was performed.Results The D value at renal artery level and infrarenal artery level of abdominal aortic aneurysm was (1.05 ±0.22)×10-5/Pa and (0.49± 0.18)× 10-5/Pa,respectively;and the corresponding pulse wave velocity was (9.68± 1.09) m/s and (14.96 ±4.01) m/s,respectively.The intraclass correlation coefficient of intra-and interobserver at renal level was 0.92 and 0.79,while it was 0.85 and 0.79 at inffa level.The Bland-Altman graph showed that the vast majority of points were within the 95% CI,the difference of intraobserver between the two positions was 0.017×105/pa and 0.010×10-5/Pa,and the difference of interobserver was 0.013×105/Pa and 0.018×10-5/Pa.Multivariate analysis of all these variables showed aortic distensibility to be independently correlated to body mass index,pulse pressure,diastolic pressure and abdominal aortic aneurysm diameter (R2=0.68).Conclusion The quantification of aortic distensibility using multi-slice spiral in patients with abdominal aortic aneurysm shows high stability.Taking into consideration of the relationship between different risk factors and the incidence of abdominal aortic aneurysm can lead to a better clinical approach.

ABSTRACT

Objective To analyze the characteristic of trauma in 246 fatal falls. Methods We collect 246 cases of fatal falls from Pudong district of Shanghai from 2005 to 2014, and then analyze the characteristics of trauma by using different parameters. Results The height in suicides by fall is higher than accidents by fall (25.7m:13.98m). In all fatal falls, head has a higher possibility in suffering trauma than other body regions (75.2%), while neck has a lower possibility (6.5%). The possibility of head trauma in accidents by fall is higher than suicides by fall (89.4%:75.2%). Conclusion Comparing with suicides by fall, victims in accidents by fall have lack of protection from limbs. Besides, the low rate of ifnding neck trauma suggests legal physicians should be more careful in examining this area.

ABSTRACT

Objective Explore the individual difference in carbon monoxide (CO) poisoning and analyze whether females were be more resistant to CO poisoning.Methods We collected 207 cases of CO poisoning deaths, and then we analyzed the Carbonyl Hemoglobin (COHb) results sorted by different sources of CO.Results The source of CO had significant impact on the fatal COHb level, but gender was not significant in affecting COHb level. In addition, it is possible that children and elders were less resistant to COHb than adults.

ABSTRACT

Objective This study aims to explore the characteristics of gender and age in six different suicide methods in urban area of China. Methods We collected 548 suicides from Shanghai, Jinhua, Wuhan, Loudi and Duyun, and then analyzed the age and gender characteristics in each suicide method. Results In the age characteristics, the mean age for jumping and hanging was oldest (about 55 years old), followed by drowning and hanging (about 49 years old), oral intake poisoning was much younger (about 49 years old), while carbon monoxide poisoning was youngest (about 37 years old). In the gender differences, male were more than females in suicides by sharp object (52:24), hanging (67:33) and jumping (55:45), while females were more than males in suicides by drowning (58:42). Conclusion The age and gender difference was eminent in different suicide methods. Carbon monoxide poisoning was the youngest age group in suicides and males involved more than females in suicides by mechanical methods.

ABSTRACT

Objective To investigate the effects of cognitive therapy combined with picture recognition training on depression after stroke (PSD).Methods Sixty patients with PSD after cerebral infarction were randomized into study and control groups.All accepted routine rehabilitation therapy.In addition,the study group accepted cognitive therapy combined with picture recognition training.The effects were assessed in terms of average reaction time and accuracy in picture recognition,and using the Hamilton depression rating scales (HAMDs) and the Barthel index (BI) before and after 4 weeks of treatment.Results The average reaction time and accuracy in picture recognition and the HAMD and BI scores improved significantly in both groups.The effects in the study group were significantly better than those of the control group.Conclusion Cognitive therapy combined with picture recognition training can alleviate depression symptoms and improve ability in the activities of daily living after stroke.

ABSTRACT

@#Objective To observe the effect of rational-emotive therapy(RET)on rehabilitation of patients with depression after stroke.Methods 62 hemiplegic patients with depression after stroke were randomly divided into the RET group and control group with 31 cases in each group.All patients were evaluated with Hamilton Rating Scale for Depresstion(HAMD)and Barthel Index(BI)before and after treatment.Results After treatment,the scores of HAMD and BI of the patients in the RET group was superior to those in the control group(P<0.01).Conclusion The rational-emotive therapy is effective to improve not only depressive symptoms but also the quality of living.

ABSTRACT

Objective To investigate the clinical, hemorheology, transcranial Doppler (TCD) and auditory brainstem evoked response (ABR) features in senile sudden deafness.Methods The clinical manifestations, hemorheology, TCD, and ABR data in 82 patients with senile sudden deafness were analyzed retrospectively.Results (1) The primary symptoms mainly included tinnitus (65.9%) and vertigo (56.1%), which mostly occurred at daytime (53.7%).(2) Most of the patients had risk factors of arteriosclerosis such as hypertension (75.6%), diabetes (59.8%) and so on. These patients had more serious deafness.(3) There was no obvious relationship between high blood viscosity (61% of the patients) and the degree of deafness. (4) 85.4% of senile sudden deafness patients indicated vertebral-basilar artery insufficiency in TCD. There was a positive correlation between blood supply and degree of deafness. (5) The abnormal rate of ABR was 100%. WaveⅠdisappeared in most patients, which showed serious damage of auditory nerve. Disappearing of waveⅠ, Ⅲ, Ⅴ and Ⅰ~Ⅲ, Ⅲ~Ⅴ, Ⅰ~Ⅴ delitescence period delaying might be complicated with the damage of the structure of the brainstem and cochlea.Conclusions The morbidity and degree of senile sudden deafness are associated with insufficiency of blood supply of internal ears. TCD and ABR are sensitive and reliable methods in diagnosis, progression and prognosis of senile sudden deafness.

ABSTRACT

The pathogenic effects of crops from Keshan disease area were studied by rat-feeding. As double controls, another two groups were fed with crops from non-endemic area and synthetic diet. Those from disease area caused necrosis of heart muscle vascular changes of myocardial mitochondria and fatty degeneration of liver. Compared with the controls, the free radical and lipid peroxide content in heart and liver were significantly higher, while the ATP and AN contents in these organs were apparently lower. The multielement contents and its constituent patterns in the feeds, whole blood, heart, liver, kidney and brown adipose tissue of rats in experimetal group were apparently different from those in the control groups. The statistic analysis of multiple factors showed that eight elements Se, Mo, Cu, Mn, Cr, Fe, Zn and Pr might be the perilous factors, that is, under the condition of low selenium, the constituent pattern of selenium and the eight elements might be the complicated factors of endemic crops.

ABSTRACT

Observations were conducted on 11 kinds of tissues and organs(brown adipose tissue, thymus gland, blood, heart, lungs, muscle, bone, kidneys, spleen, liver, and intestine) in experimental animals, which were fed on crops from endemic areas of Keshan disease and non-endemic areas for 8-13 weeks. The absorption and distribution of 75Se (Na2 SeO3) in above tissue and organs were studied. Results revealed that the absorption of Na2-SeO3 in experimental group fed on crops from endemic area was higher than control group fed on crops from non-endemic area. The gradient of distribution of 75Se in various organs was regular: The highest absorption was in kidneys, the lesser was in liver, spleen and thyrnus, the least was in muscle, heart and bones.In one week observation, the contents of 75Se changed as time passed, but the fluctuating changes of the two groups were different.The regular differences of 75Se absorption as above, revealed a distinction between endemic and non-endemic crops. Under the influences of the crops on experimental animals, there appeared more or less a condition of selenium deficiency, which proved that the soil-water-food chain had apparently influenced the selenium metabolism of the organisms.
